Registering Your Child

The following documents are required for registering your child:

  1. Birth Certificate
  2. Up to date immunization record.
  3. Proof of Residency - Example: rental agreement, mortgage papers or a utility bill (SRP or City of Phoenix water bill in your name).
  4. Withdrawal form from previous school.
  5. Current report card from previous school.
  6. AIMS reports or state mandated testing reports.
  7. Any discipline reports your child may have.

If you plan to withdraw your student from Western Valley Middle School:
Please notify the office at least the day before your student's last day to prepare the paperwork. To withdraw your student, you'll need to:

  1. Sign a withdrawal form.
  2. Return books/pay for lost books and/or materials.
  3. Clear all outstanding debts. 
  4. Please bring your state identification upon withdrawing your child.
